The Case Manager, under supervision is responsible for providing support and wellness/recovery resources to assist clients in reaching the optimal level of wellness and functional capability. The case manager is a core member of a collaborative care team, providing direct billable support and coordinating services 65% of the time.

 The ideal candidate has:

  • Bachelor’s degree in psychology or a related field, or equivalent training and experience 
  • One year or more of case management related experience.
  • Familiarity with community-based resources.
  • Understanding of cultural issues, community resources, and family dynamics.
  • Valid California driver license
